Unless previous arrangements have been made in advance by our patients, copayments, coinsurance and any outstanding balances are expected to be paid at the time of service.

Patients may be financially responsible for payment for all services, even if your insurance company does not cover the service. Patient accounts that are not paid promptly are subject to third-party collections and legal procedures.

Please notify the front office staff if your insurance has changed since your last visit.

Please check your insurance for information about referrals, copayments and any deductibles required at the time of your visit to our clinic.

We accept cash, personal checks, Visa, MasterCard, Discover and American Express.

For your first visit, please bring your insurance card and arrive early to complete the necessary patient information forms.
